Use this form to send us:
- Your contact info, dates attending, and volunteer options
- Parent permissions for children, emergency contacts
- Children’s ages and emergency contacts
Please fill out the form once for EACH PERSON attending in your group.
Children under 17: what to do if a child is attending under the supervision of someone other than a parent
Each PARENT must fill out the emergency contact/permission form for minors who are attending under the supervision of someone other than a parent.
If someone else has already filled out the registration info for your child:
- use this form to fill out the permission form
If you are filling out the registration form for your child, no other action is necessary - it is part of the registration.
STEP 2: Lindenwood Registration Form
Use this form to make payments for your rooms, meals and conference fees.
Deadline for Lindenwood Registration: June 30, 2020
This is a FIRM deadline, to accommodate ordering food for meals.
TIP: If you create an account during checkout, you’ll be able to view your order later and add to it, but not change or delete items. See Step 3 below for change details.
Rooms
All rooms include include bed linens and towels and have private bathrooms (no tubs).

* Note for families:
- Double rooms in the residence halls where you will stay have 2 twin beds. For a one time fee of $20 per bed, 2 extra beds can be bunked per room.
- You can bring your own cots, pack-n-plays, air mattresses, etc for no extra charge. There is plenty of floor space.
More details
- We will assign the double/single rooms at the Lindenwood Retreat Center, a building separate from the residence halls, for people who may relish a little quieter experience. If these spaces are full, single or couples will be assigned a room in a ‘quiet zone’ of the residence halls. Single/double prices are the same regardless of location.
- If you need special accommodations or would like to be close to other family members, please indicate on the Lindenwood registration form by using the "Anything else you want to tell us" box.
- Food may be brought into the rooms, however there are no refrigerators or microwaves.
- Washer and dryers are available free of charge in the student residences.
- If you are sharing a room with someone, please arrange that before you make your room reservation, so you will fill in the Lindenwood registration form only once for both people.

Meals
$20 per day (if eating 3 meals). Children 3 years and younger eat free.
Or - chose individually to meet your needs.
- Breakfast: $6
- Lunch: $6.50
- Dinner: $7.50
In the registration form, you’ll have the opportunity to enter any special dietary needs you may have. Lindenwood will try to accommodate them as best as possible.
Conference fees
These are collected in the Lindenwood registration form for anyone attending the conference.

Note: if you are only coming for one day, the rate is $30 for anyone 4 years old and over.
Commuters / camping / hotels
Camping and hotel accommodations are nearby in Plymouth. Please explore these on your own. Conference fee and any meals you choose as a 'commuter' can be paid through the Lindenwood registration form.
Donations
If you are able to help someone financially who might otherwise not be able to attend, here are some ideas!
- $60 - conference registration for an adult
- $80 - food for a family of 4 for a day
- $166 - lodging and meals for a family of 4
- $180 - conference registration for a family of 4

STEP 3: Yikes! I need to make a change or I missed the registration deadline
Life brings surprises and you might need to change an already submitted form or found you can attend when you thought you couldn’t.
- To change information entered in the Focolare form or let us know your attendance dates have changed, please email Midwest.mariapolis@focolare.us.
- To make a room or meal change BEFORE the June 30 deadline, please contact Lindenwood directly at lindenwood@poorhandmaids.org or call (574) 935-1780.
- To make a room or meal change AFTER the June 30 deadline, contact midwest.mariapolis@focolare.us to see what can be arranged.
Refund procedures and deadline
- Full refunds can be given through Lindenwood until June 30, 2020 (574-935-1780).
- After June 30, changes will need to be arranged and/or payment may be transferred to others. Please contact midwest.mariapolis@focolare.us.
